Emergency Preparedness and Transition Activities within the Regional Collaboratives

Presenters will discuss tools developed by three HRSA Regional Collaboratives to aid families and providers in emergency preparedness or transition activities. The Region 4 Genetics Collaborative will present information on MyEIF.org, a web-based tool used to summarize and communicate critical personalized information during a medical emergency; the New York-Mid-Atlantic Consortium will discuss Emergency Cards for conditions identified through newborn screening and evaluation of the Minerva Health Record, which can be carried on a flash drive; and the Western States Genetic Services Collaborative will present a portable health recorddesigned for individuals with genetic and metabolic conditions.
