Family Health History from the Ground Up – Web 2.0 and Community Outreach

Community-based work, like everything in the 21st century, must embrace technological innovation. The Community Centered Family Health History Project, with 20 partners across the country, exemplifies the combination of traditional grassroots, door-to-door style efforts with web 2.0 presence. In this presentation, participants will learn about the successful integration of family health history (FHH) information into existing programs and will hear from a project partner about challenges in the process. Participants will jointly customize FHH resources using the Does It Run In the Family? online tool.
