Community-based work, like everything in the 21^st century, must embrace technological innovation. The Community Centered Family Health History Project, with 20 partners across the country, exemplifies the combination of traditional grassroots, door-to-door style efforts with web 2.0 presence. In this presentation, participants will learn about the successful integration of family health history (FHH) information into existing programs and will hear from a project partner about challenges in the process. Participants will jointly customize FHH resources using the Does It Run In the Family? online tool.
